Out of Hospital Management of Medical Conditions
Building on the previous modules and practice experience, this module will enable learners to enhance their understanding of pathophysiology and develop skills to effectively manage community based care. The module will facilitate learners to undertake a safe consultation that includes the broader issues of health and social care, enabling appropriate clinical decision making that meets the needs of the patient and their relatives or carers. Leaners will develop their understanding of health promotion, appropriate pathways of care and enhance their skills towards effective and safe management of patients within the community setting.
Developing problem based learning will be facilitated using a blended approach for professional development; with opportunities for learners to work and learn within an interprofessional setting.
Practice based learning will be supported by a week placement in a hospice and a further week placement within the community supported by Palliative and End of Life Care teams. Further placements will be undertaken on an emergency ambulance and or rapid response vehicle with the Welsh Ambulance Service.
The module will be supported by e- learning.
