Book an appointment - To book an appointment please contact CIF on 01792 606420 or email your referral. We require a referral from a registered medical professional for all scans. 

Referral Forms

We require a referral from a registered medical professional for all scans. Without a referral form, we will be unable to facilitate your scan. Both MRI and CT referral forms can be found below:
